LPN Clinical Coordinator

ORGANIZATION
This position reports to the Director of Clinical Services

ESSENTIAL FUNCTIONS includes the following.  Other duties may be assigned.

The LPN Clinical Coordinator is responsible for daily completion of the following tasks: Referral Processing, Ongoing Orders, Workflow, Escalation/Complaint Calls, Updating Clinical Information, Reporting, Verbal Orders, and Field Visits as required.

The LPN Clinical Coordinator performs clinical preparation of key office functions: Stand up/stand down & case conference that ensure compliance KPIs (LUPAS, Paraprofessional Utilization, etc.).

The LPN Clinical Coordinator ensures services follow professional standards, state, and federal regulatory requirements.

The LPN Clinical Coordinator complies with applicable legal requirements, standards, policies, and procedures including, but not limited to the Compliance Program: Code of Conduct, HIPAA, and Documentation Standard

The LPN Clinical Coordinator assists in the selection, training, scheduling, orientation and competency validation of designated personnel.

The LPN Clinical Coordinator supervises, plans, develops and organizes services and activities to support medical practices and performs a wide variety of patient care activities and accommodative services to support the medical practice.

The LPN Clinical Coordinator is responsible for coordination of faxes for clinical staff and ongoing management faxes.

The LPN Clinical Coordinator develops and monitors plan for catheter change, blood draws, labs, wound care and assists with retrieving results for ordering physician.
 
KNOWLEDGE & EXPERIENCE
  • 1-2 Years of experience as a Clinical Coordinator
  • Current state licensure in Oklahoma and in good standing per state requirements.
  • Current Basic Cardiac Life Support (BCLS) required
  • Current Advanced Cardiac Life Support (ACLS) preferred
  • Certified Rehabilitation Registered Nurse (CRRN) preferred
  • 1-2 years recent Home Health nursing experience
  • Active Oklahoma driver’s license
  • Current car insurance and registration
  • Required vehicle
  • US Citizen, work sponsorship unavailable
  • Medical knowledge, patience, compassion, kindness, critical thinking skills, knowledge of infection control practices, stamina, and physical endurance, observational skills, knowledge of occupational hazards and safety rules, communication skills, leadership, empathy

MACHINES & EQUIPMENT
The LPN Clinical Coordinator must be able to effectively operate the current computer system, telephone system, and other office machines such as a copier.

PHYSICAL REQUIREMENTS

  • The LPN Clinical Coordinator is required to perform moderately heavy physical exertion on a regular and recurring basis such as: extensive driving, assisting patient in transfer activities (wheelchair, to bed, to tub, to commode) and providing substantial support to individuals in ambulation.
  • While performing the duties of this job, the LPN Clinical Coordinator to talk, hear and use hands to feel and handle.
  • The LPN Clinical Coordinator frequently is required to stand; walk; reach, sit, stoop, kneel, crouch or crawl.
  • The LPN Clinical Coordinator must occasionally lift and/or move over 100 pounds.
  • Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

WORKING CONDITIONS
The work environment characteristics described here are representative of those a LPN Clinical Coordinator encounters while performing the essential functions of this job. Settings include, but are not limited to; General office, patient home setting and automobile.

TRAINING
Security and privacy training will be provided as part of this role to protect the confidentiality, integrity, and availability of PCS’ and PCH’ associate and customer data.
